Rates<\/h3>\n

Please contact our office and request an initial consultation if you have any question about our fee schedules. Please be advised that your out-of-pocket amount will be determined by the actual insurance benefits according to the conditions such as the portion of your deductible already paid or co-pay amount to be paid.<\/p>\n

Billing<\/h3>\n

As valid payment methods, we accept all major credit cards (Visa, MasterCard and American Express) and debit cards, as well as checks. Due to our policy, you are required to pay the fee when your consultation or treatment session is complete.<\/p>\n

Insurance<\/h3>\n

Please take note that Dr. Matsuki is currently not listed as an in-network provider for any insurance panels. Our office will submit your claims to your insurance company for reimbursements if out-of-network benefits are available through your insurance plan. An estimated financial responsibility is calculated from specific benefit levels on your active insurance coverage, and provided to you before any treatment. Our office is always willing to help, so please contact us for assistance. We also contact your insurance company if a pre-authorization is required before receiving our service.<\/p>\n

Cancellation Policy<\/h3>\n

As a rule, two business days\u2019 (48 business hours\u2019) notice is required to cancel or reschedule your session for whatever reasons. For example, our office must be notified on or before the preceding Friday if you want to cancel or reschedule an appointment for Monday. Failure to cancel and miss a session, or cancellation of a session within 48 business hours, will result in paying the full amount of the appointment fee due to our cancellation policy.<\/p>\n<\/p><\/div>\n
